Creating & Uploading OASIS Assessments

Creating an OASIS File for Submission

  1. Open the OASIS Console in HCHB.
  2. Click the Create Files button to pull up all patients with a Locked OASIS assessment.
  3. An individual or multiple assessments may be submitted.
    • Agencies – If you are sending for a specific patient, check the box for only that patient
    • Med Recs – If you are submitting for all offices, select the top check box to highlight all patients in the list
  1. Once you have checked the boxes of the assessment(s) to be sent, click the Create Files and Mark Sent button to create the OASIS transmission file. Click Ok on the pop-up stating the files were created.
  2. Click on Close to exit the OASIS dialog box. 
  3. A file will be created and sent to the folder indicated in your settings. The name of the file is based on the date/time the file is saved and NPI.

 

 

 

 

Submitting an OASIS File

  1. Navigate to the iQies website (https://iqies.cms.gov/) & log in.
  2. If it does not already take you to the “Upload Assessments” page, select Assessments at the top and then select Upload Assessments.
  3. Select the blue Upload Assessments button.
  4. Browse or Drop/Drag your files created from HCHB then click Submit.

5. A green banner will let you know if it was successful, but the upload can take a few hours, so check back the later that day or the next to pull the status report.
