Medbridge Attestations - Legal name is incorrect
With the Medbridge Attestation forms, it requires an exact match to the employee's name as it appears in Medbridge.
When entering the name in the phone, it will prompt to auto-populate since it is a known word for a user's phone. When doing this, it automatically adds a space after the name. This will cause an error since it won't match exactly.

To fix this: simply remove the space after the prompted word.
